Information sheet to be completed by parent/guardian for each camper. This confidential information helps our staff meet your child's needs for the summer. Please fill out completely, sign and return.

Individualized Orders

The upper portion of this form must be completed for each camper by your physician, and filled out and signed by them at the bottom. If your child will be taking any prescription medication while at camp (including medication for overnights), this portion must also be completed by the physician. Camp nursers are only permitted to dispense medications that are approved by your physician. Note: Nurses will contact you before dispensing any over-the counter medications. ALL campers should have this form on file.

Car Pool List

Forms

Complete this form if you will be setting up a permanent carpool situation with another camp family/families. All families in a carpool must submit a form. Children will be assigned to the carpool pick-up area for the entire session. PLEASE NOTE: All campers in Pre-K, K and 1st grade, along with their siblings, will automatically be assigned to carpool dismissal - no form required unless you are also carpooling with another family.
